U.S. PASSPORT APPLICATION GUIDELINES
Passport services are available up to ½ hour before the Library closes
APPLYING FOR A PASSPORT?

Bring the following :

  • PROOF OF U.S. CITIZENSHIP (original non abstract certified birth certificate, or your old or current passport, or your naturalization certificate)
  • AGES 18 AND OVER NEED PROOF OF IDENTITY (valid driver's license, state I.D., passport, or Citizenship Certificate )
  • TWO IDENTICAL PASSPORT COLOR PHOTOGRAPHS (2 in. x 2 in.) (Passport photos are now available at Placentia Library District for $10.00)
  • 1 PAYMENT TO Passport Services (check or money order only)
  • 1 PAYMENT TO Placentia Library District (cash, check, credit card, or money order)
  • DS-11 Form – complete section 1–20 only (DO NOT SIGN) (you can also download forms at www.travel.state.gov)
  • Children under 16 – both parents must be present with child or submit a notarized statement of consent for parent unable to appear. Parents must also have photo I.D. An original birth certificate is needed for new or renewed passports.
  • Children ages 16-17- at least one parent must be present with applicant. Parent must have valid photo I.D. Note: All applicants must be present.
  • Effective 2-1-2008 the U.S Department of State will be offering a passport card for travel to Mexico and Canada by land and sea. Fees will be $45 for a ten year card and $35 for a five year card.
Applicants Pay to Passport Services with check or money order only Pay to the Placentia Library District with cash, check, credit card, or money order Total Passport Fee:
Age 16 or over $75+ $25 =$100
Age 15 or under $60+ $25 =$85
(Allow 6 weeks for the return of the passport for regular service)

EMERGENCY PASSPORTS Travelers departing within 2 weeks must make an appointment with the Los Angeles Passport Agency at 1-877-487-2778 (24-hour appointment scheduling system)

PASSPORT RENEWALS:
Most renewals can be done by mail–see form guidelines. Renewals less than 5-years expired are done by mail. DS-82 renewal forms are available at the Circulation Desk. You can also download forms at www.travel.state.gov

EXPEDITED SERVICE: If travel is in less than 6-weeks: $60.00 extra Passport Services Fee per application. Delivery within 2–3 weeks. PLUS (If Desired) Express mail postal service: $16.50 will return your passport plus other documents via express mail rather than priority mail
